Canceling Time Off Requests

  • If an employee has put in for time off and needs to cancel the request, they may do so at least 30 days prior to their requested start date of PTO

  • Last minute cancellations may still have the employee off on the days requested, due to a relief service being used for coverage and their cancelation policy

  • A PTO Cancelation form should be filled out, and can be found in the cascading file folder on the wall by the side door

  • PTO Cancelation forms should be submitted to the Practice Manager via the hanging file folder organizer on the closet door in the office
