Canceling Time Off Requests
If an employee has put in for time off and needs to cancel the request, they may do so at least 30 days prior to their requested start date of PTO
Last minute cancellations may still have the employee off on the days requested, due to a relief service being used for coverage and their cancelation policy
A PTO Cancelation form should be filled out, and can be found in the cascading file folder on the wall by the side door
PTO Cancelation forms should be submitted to the Practice Manager via the hanging file folder organizer on the closet door in the office
