Canine Export Form Antech

This form is to be used when Antech requires a Canine Export Form - it will either let you know on the Antech website the form is needed, or a rep from Antech can tell you if you're unable to find it. Typically, these are used for exporting to other countries and will almost exclusively be used for health certificate testing depending on the country.  This form should go in the bag with the accession if you have confirmed with Antech that the test requires it. They will disburse to appropriate labs from there with this form.
