Patient weight and TPR should be captured in the MCR for each pet that is seen in our hospital

The only exception is for pocket pets that are just in for nail trims, and pets that are only here for anal gland expression

Subjective notes should always be time stamped prior to entering information

Doctors shall write their notes in the records in CAPITAL LETTERS and initial at the end of their findings
