Boarding Sheets

  • Boarding sheets are to be filled out in their entirety for each pet that stays overnight at MAH

  • These sheets are available at the front desk

  • Reception will fill out the initial sheet(s) according to their travel dates at check-in

  • All belongings should be inventoried and placed on the boarding sheet

  • Make sure colors of items are noted under belongings

  • If the pet comes in without a leash, or carrier, please notate that on the boarding sheet and in the MCR Subjective notes

  • Should the check-out date be extended, it is the kennel attendant's responsibility to add the appropriate days to the boarding sheets, or create new sheets to accommodate additional days as necessary

  • Any person doing kennel rotation should be initialing feedings and medications

  • Medication administration should also be put in the computer as AM or PM medications for each time period the pet is medicated

  • If medications are added while pet is in our care, the assistant who worked with the doctor to examine the pet shall add the prescribed medications to the boarding sheet and notify the kennel attendant(s) that there is a change to the boarding sheet
