Surgical Discharge Sheets

Discharge sheets for dog and cat spay/neuter procedures, as well as rabbit spay/neuter procedures, are linked in the Avimark drive

Right click on the surgery line item in Avimark and click Form - Edit and select Yes when prompted.

Search for the appropriate discharge and double click to attach

Include medication instructions and any special discharge instructions. Most diagnoses will be "Healthy Pet" unless noted by EJO

Anything in an angle bracket (< >) will change automatically with the merge words inside of them, so there is no need to edit those areas (doctor name, pet name, etc)

Save the file once you're finished editing

Go back to the line item, right click, form, print

The page should print with all of the correct information, including the merge words to indicate the pet's name
