• Fill out form with all information that is needed for the sample, including owner information and patient information, as well as history (can be obtained from MCRs at diagnosis and SX information) 

• Histopathology, Biopsy

• Go to https://www.247cslx.com/clientportal

• username, mcdonoughah    password 123Jonesboro!

• New Order tab

• Verify the information is correct (from, sender,etc)

• Click Continue

• Submit Order

• Print label

• Package specimen in clear UGA specimen bag, slide submission form and shipping label into the back pocket of the bag - shipping label facing outward

• Leave up front for the courier
