Workplace Injuries

• Have employee administer first aid to themselves if applicable

• OSHA Form 301 should be filled out for the incident. The form is located in the white binder on Amanda's book shelf titled "WORKER'S COMP" - 301 is under the tab for Form #4

• Ask employee if they would like medical treatment outside of this facility

• If they decline, at the bottom of the 301 form, write "staff member declined medical treatment"

• Have employee sign and date their 301 sheet at completion

• If the employee wants to be seen by a provider, they should go to one of the following places:

If using Peachtree Immediate Care, take the form at the end of this page in addition to the other two. 

• Recommended to make an appointment for the employee, or have them make it themselves, to save time.

• Bring the Prescription ID Card and the Workers Comp Packet linked below

• *Note - bring vaccine records of pet that caused injury if applicable
